A change package is a collection of evidence, benchmarks, and promising ideas that practices can use to improve patient care. These concepts and ideas are applied to a particular improvement goal in a specific context. 

These resources were developed by the AAP Quality Networks as part of a large-scale quality improvement project in acute and primary care settings. Some clinical resources may reflect office processes and policies that differ from that of your own care team or practice and will require tailoring to meet the needs of your specific patient population and care team environment.
