Brief Description
Fit Together is a partnership between a pediatric primary care or weight management clinic and a local Parks and Recreation department. Children aged 5-12 with obesity are cared for by their clinician, and families participate in intensive lifestyle activities in an inclusive, community-based setting. The Fit Together curriculum, called the "Playbook," includes all the tools necessary to build the partnership, complete agreements, develop a referral system, hire staff, deliver the Fit Together model, and sustain the program over time.

Fit Together is 6-12 months, better outcomes the more engagement there is.
