This webinar provides an overview of CHD Toolkit Resources and how they can be used in practice, and features a multidisciplinary discussion of important considerations for care planning, care coordination and cardiac care referrals for patients with Congenital Heart Defects (CHD).

- Identify opportunities to reduce gaps in care for individuals living with a congenital heart defect
- Increase physicians’ awareness of the importance of lifelong care for patients with CHD by demonstrating CHD toolkit resources for physicians
- Promote knowledge of intervention, coordination strategies and referral practices for patients with CHD by discussing a case study
Presenters
Jeanne Van Cleave, MD, FAAP
Dr. Jeanne Van Cleave is an Associate Professor of Pediatrics Children's Hospital Colorado with an interest in care for children with medical complexity and children and youth with special health care needs, optimizing care plans and improving care coordination for adolescents and young adults. Dr. Van Cleave is a leadership team faculty for the Optimization of the Pediatric Medical Home for Children with Congenital Heart Disease QI Learning Collaborative and a member of Council on Children With Disabilities (COCWD).
Michelle Gurvitz, MD, FACC
Dr. Gurvitz is an assistant professor of pediatrics at Harvard Medical School and a staff cardiologist with the Boston Adult Congenital Heart program at Boston Children’s Hospital and Brigham and Women’s Hospital. Dr. Gurvitz is board certified in pediatrics, internal medicine and pediatric cardiology and has a master’s degree in Health Services Research. Currently, Dr. Gurvitz divides her time between research, clinical care and administrative activities. Dr. Gurvitz also sits on a number of committees and advisory boards regarding care of adults with CHD and maintains an active clinical practice.
Afshan Hameed, MD, FACOG, FACC
Dr. Afshan Hameed is a board-certified University of California-Irvine Health cardiologist and maternal-fetal medicine specialist. She is a nationally recognized expert who co-authored the American College of Obstetricians and Gynecologists' inter-conception care guidelines for women with hypertension to improve postpartum care and follow-up. Her clinical interests include heart disease in pregnancy, fetal echocardiography, preeclampsia and anticoagulation in pregnancy.
