Although most individuals with a Congenital Heart Defect (CHD) require lifelong cardiology care, many adults are lost to cardiac follow-up or may experience gaps in care. This video-based educational curriculum aims to improve continuity of care between primary care and specialty cardiac care physicians for patients in care and mitigate adverse outcomes for patients who have been lost to follow-up when for instance, they may present in the emergency department or become pregnant. These quick, yet in-depth videos, will aim to provide real-time education to improve knowledge with respect to advances in CHD care for children, youth and adults, and support physicians in their care for children, youth and adults living with a CHD.
