Description

Chronic health conditions affect more than 14 million school-aged youth (almost 20% of the school-aged population), with approximately half of these being moderate or severe. Physicians and other health professionals should use the experience and expertise known about health issues as they relate to schools. This course will emphasize how care coordination has been shown to improve patient, family and health system outcomes.

Learning Objectives

  • Identify current chronic condition management frameworks for school, pediatric provider, and parent collaboration.
  • Identify best practices for effective partnership and communication between schools, families, and pediatric providers in chronic condition management.
  • Select best practices from chronic condition management frameworks and communication partnership practices and apply them successfully to given case study scenarios.
