Community Pediatrics Self-Assessment

Purpose
The Community Pediatrics Self-Assessment was designed to help practicing pediatricians assess where they are in relation to community pediatrics activities as recommended in the COCP policy statement, The Pediatrician's Role in Community Pediatrics. The Self-Assessment provides an opportunity for reflection about how individual pediatricians practice with a community focus. It is intended for pediatrician in all types of practices, in all communities, and in all stages of their career.

Overview
The Self-Assessment contains six Key Factors that exemplify the practice of Community Pediatrics as defined in the policy statement.  They include:

• Practices with a population-based approach
• Communicates/collaborates with external key stakeholders/partners

• Considers the social determinants of child health
• Uses community resources to enhance children’s care
• Advocates for patients/children as individuals and as a group/community
• Values evaluation, research, scholarship, and education

Each Key Factor has anywhere from 1-3 Themes.  These Themes are represented with a progression of activities and are expressed as a continuum from Component 1 – Component 3.

The Community Pediatrics Self-Assessment is currently available as a pdf download. The COCP plans to make the Self-Assessment available as a completly web-based instrument in the future.

Pilot Test
In the summer of 2007, the COCP conducted on online pilot test. The results of the pilot test are available as a pdf download if you would like to see how your results compare to other pediatricians who consider themselves community pediatricians.

Community Pediatrics and the Medical Home
The COCP recognizes that the concepts of community pediatrics and medical home are inherently linked. Many of the concepts in this Self-Assessment directly reflect those in the policy statement "The Medical Home". The structure and format of this Self-Assessment is based on the Medical Home Index developed by the Center for Medical Home Improvement at Crotched Mountain in Greenfield, CT.

If you are already a member of the AAP and would like to become a COCP member, join online by:
1. Go to http://www.aap.org/moc/indexEntry.cfm and login with your AAP ID and password.
2. Click on "Member Community".
3. Click on "Join a Section or Council" under Section, Council & Committee
Information.
4. Choose "Council on Community Pediatrics", answer a few questions, and click "Submit".
